Filing a Claim

If you feel you have incurred damages caused by the City, please complete this form and return it to City Hall or email the form and all supporting documentation here. Once the paperwork is processed, expect to receive a letter from Carl Warren, the City’s claims management firm. They will investigate the claim and communicate with you throughout the process.
